    牙龈外露一直是矫正治疗中的难症，有许多的患者因为寻求单靠矫正来解决该问题，而惨遭医师拒绝。笑起来露牙龈的原因有许多，举例来说上颚骨垂直的高度过长，和骨骼过于突出是主因。所以矫正治疗中需要拔除上颚小臼齿，以让前牙后移。我们利用迷你骨钉作为锚定作用，以让上颚牙齿上移，并辅以牙周手术，去除过多的牙肉以让牙齿形态更为完美。

    In the following patient, her gummy smile problem is from vertical too-long skeletal structure. Too much forward projection of the maxilla bone is also noticed. Therefore, we decide to extract upper teeth to retract the upper jaw and push the upper teeth upward by miniscrews as anchorage.     如果只是考虑上颚前牙的上提或压入，而忽略掉其实齿列矫正必须同时也将后方牙齿上移，否则就会产生『前牙开咬』的现象，『开咬 Open Bite』的副作用。所以齿颚矫正牙龈外露，绝非单纯而又容易的事。再将前排牙齿上移的过程，同时必须注意是否左右边的牙齿切端是否有对称，是否有形成理想的曲线。
